Wedding Policy Acknowledgment (WPA)
This acknowledgment should only be submitted AFTER we
have agreed to hold a date for your Wedding on our calendar.
ALL INFORMATION IS REQUIRED. Do not submit an incomplete acknowledgment unless it is
explained in the "Comments" section. Incomplete submissions may delay confirmation of your Wedding date. It may be
possible to arrange for more time by notifying the Basilica office, if no alternate hold has been placed on the
same date for another couple.
We require a separate submission of the following
form from the Bride and the Groom. These completed forms will be the beginning of your wedding
file. To help us serve you better, please let us know at your earliest convenience, when any of the submitted information
changes.
Once the WPAs are submitted, a computer generated confirmation should appear.
A Confirmation Packet will be mailed to you within two months after we receive your WPAs and the non-refundable
deposit. Until we receive both WPAs and the deposit,the tentative Wedding date cannot
be considered Confirmed.
IMPORTANT: When completing the form, be sure to use the TAB KEY between questions.
Pressing the ENTER KEY will prematurely submit form.
